What you need to know

• You can only fill in this form if you're a person or organisation referring another person (a client)
• £500 is the most that can be awarded
• one application per calendar year is permitted per client
• the funds cannot be paid to the client

What you'll need

To refer your client for the mayors local support fund, you'll need:

• a supporting letter
• quotation, invoice or screenshot of desired items
• a bank account name, number and sort code or internal cost code
• details of everyone living in the household (including lodgers)
• information about previous applications or help with money
• permission from the client to use their personal information
