Acupuncture and cosmetic treatments registration - workplace health and safety
You must be registered with us to offer acupuncture treatment, tattooing, semi-permanent skin colouring, cosmetic piercing and electrolysis in our borough. Find out how to register.
Businesses and practitioners who provide acupuncture treatment, tattooing, semi-permanent skin colouring, cosmetic piercing and electrolysis are required by Section 14(7) and Section 15(7) of the local Government (Miscellaneous Provisions) Act 1982, viewable from the Downloads section, to register with us if they wish to operate in the borough.

To register as a new business or practitioner, you will need to send us:
a completed application form for
Piercing, Tattooing, Acupuncture, Electrolysis and Skin Colouring premises registration [78.1KB] or
Piercing, Tattooing, Acupuncture, Electrolysis and Skin Colouring personal registration [525.05KB]
copies of any training certificates
a copy of a valid insurance policy
a copy of a Sharps Waste Collection Agreement
a copy of your client medical questionnaire
payment - see Fees, below.
Documents may be submitted by post or emailed to regulatoryservices@guildford.gov.uk. We cannot accept photographs of documents, only clear scans.
Fees
To license a premise - £201.00. This fee includes the licensing of one practitioner.
Additional practitioner for an existing licensed premise and who has received a visit from the Food and Safety Team within the last 12 months - £93.00 per additional practitioner.
Inspection
Once we've received your completed application, a member of the Food and Safety Team will arrange for an inspection.
You should anticipate a visit shortly after your application is submitted.
